I've lost my Theory Test Certificate?

Booking a test

You generally don't need your Theory Certificate number when booking a test. However, if you are asked for it, but can't find it, you can contact the DVSA.

To get the info you require, you will need to give:

  • Full name as it appears on your driving licence
  • UK driving licence number
  • Date of birth

Car Driving Test

You don't need a replacement Theory Test certificate to take the car driving test.

Before the test begins, your examiner will be able to check whether you've passed the Theory Test.

Other Driving Tests

You will need to contact the DVSA with your:

  • Full name as it appears on your driving licence
  • Address
  • UK driving licence number
  • Date of birth

A letter will be sent to you that you can take to your driving test and use as proof of passing the Theory Test.

I've lost the provisional licence counterpart, what should I do?

The 2 most important things you need to take to your driving test are the provisional licence and paper counterpart - without both of these, you won't be allowed to take a driving test. If you have lost either, you will need to apply for a duplicate through the DVLA as soon as possible (a duplicate will cost £20).

The quickest way to apply for a duplicate is to ring the DVLA duplicate service on 0300 790 6801 (they will accept card payment - Visa, Eurocard, Mastercard, Solo, Electron and Maestro). You could also apply for a duplicate online or by completing and sending the Form D1 (available at the Post Office) to the DVLA.

I've lost the Theory Test booking number?

If you ring the customer service team at the DSA responsible for the theory test on: 0300 200 1188. They will be able to give you your booking number. To cancel or change your theory test date without losing your fee you will need to ring the theory test booking line on 0300 200 1122 no less than 3 days before the test date.

Lost provisional, do I need my driver number to get another?

Providing that none of your details have changed, you will need to give your driver number OR full name, date of birth and also confirm your UK address.

If your photocard and paper counterpart have been lost or stolen, you can apply for a duplicate licence by telephone using a credit or debit card. The fee for a duplicate licence is £20.00. The DVLA accepts Visa, Eurocard, Mastercard and Maestro. To use this service ring 0870 240 0009, Monday to Friday 8.00 am - 8.30 pm or Saturday 8.00 am - 5.30 pm.

I've lost my passport, can I use other ID for my theory test?

Normally you would take your Theory Test appointment letter, the provisional licence and paper counter part for identification. If you have an old paper licence without a photograph a valid passport will be required - no other form of identification is accepted. Photographic identification is required to stop tests being taken by someone pretending to be the candidate. Unfortunately, a valid passport is the only photographic ID accepted.

Lost Provisional Licence
Will I be able to take the test?

In the first instance, if you've lost your provisional licence counterpart, we would suggest changing the test date (if you have 3 CLEAR WORKING DAYS before the test date, you can do this without losing your fee) and then getting a replacement provisional and counterpart.

However, many candidates contact us with less than 3 days until their test.

Officially the DSA say that you need to take BOTH parts of the provisional licence along to the test or it will be cancelled with loss of fee. The reason behind this is fraud - the DSA are trying to crack down on people taking someone elses Theory/Driving Test for them.

However, there has been cases where the candidate was only asked to show their provisional photo licence card and not the paper counterpart (it's up to the DSA official at the test centre to ask for the ID).

There has even been a few cases of candidates being allowed to take the test after pleading with the official at the test centre (again, these candidates had the photo licence card but not the paper counterpart).

Therefore, logic would say that there's a very slim chance that you might be able to take the test with only the paper counterpart and suitable photo ID. But be warned, most DSA officials will follow the correct procedure which would mean the test won't go ahead.

However, if the fee will be lost anyway, many candidates decide it's worth a try.
